For now, you'll have to download the application,
fill it out, scan it, and attach it to an E-Mail.

Unless you have a program that can edit pictures.
Then you should be able to fill it out with that program, Save it,
an attach it to an E-Mail.

Each page is sized to be printed on a standard 8 1/2" X 11" piece of paper
